If you have a blood pressure machine at home and prefer to do your own readings or know that you suffer White Coat Syndrome please let us know your results! This helps keep your records up to date and ensure we are optimising your blood pressure especially if you are on medication.
Please print and fill in the attached document and hand it in to reception who will pass it on to a clinician for review.
